Please fill out the online registration form at the link below. We ask for your name (as you prefer it on your nametag), your institutional affiliation, and your email address.

The banquet will take place Friday evening. Attendance is by invitation only. Tickets are available if you are bringing a guest to the banquet. Please pay $50 at the department or at the conference registration for your guest.

Please make your menu selection on the registration form.
